Here are a few reminders about medications:
1. All medication must be in it's original container (prescription or over the counter). If it is a prescription medication, it must be properly labeled with the student's name, medication name and correct dosage.
2. Students are not allowed to carry medication with them at any time. The only exception is a student with asthma, they may carry their inhaler (an asthma action plan must be filled out and signed by their physician). Parents will need to fill out the medication permission form and bring the medication to the school.
3. For your convenience, the Medication Permission form, the Allergy Action Plan and the Asthma Action Plan are attached to the right, under my picture. Please feel free to print the forms you need, have your student's physician sign them (if necessary), and bring them to the clinic along with medication. I will return Monday, August 15, 2016. You may bring any medications the week before school starts.
